If you change your address, telephone number or surname please inform the reception staff as soon as possible.

HOW DO I....
OBTAIN A REPEAT PRESCRIPTION?

If your doctor agrees, you may obtain repeat prescriptions from reception. You will be given a computerised list by the doctor. Please allow 48 hours for this service and, if you send your request slip by post, enclose a stamped, self-addressed envelope. To avoid mistakes and misunderstandings, requests for repeat prescriptions will not be accepted over the telephone.

HOW DO I....
REGISTER WITH THE PRACTICE?

Please come along to medical centre and bring your NHS medical card with you if you have it. All new patients registering or re-registering with the practice are asked to attend the practice nurse for a new patient health check. You will be asked information about your past and current health. This may be useful until we receive your previous records from your previous GP. Sometimes this can take months.
