CONFIDENTIALITY

We ask for personal information so you can appropriate and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by members of the practice team.

FREEDOM OF INFORMATION

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the “classes” of information the practice intends to make routinely available. An information leaflet is available at reception.

SUGGESTIONS/COMPLAINTS

We are happy to accept suggestions. If you have a problem with any of our services or wish to make a complaint, please ask to speak to the practice manager or put your complaint in writing and it will be dealt with swiftly by the practice manager or one of the partners.

WHAT TO DO AT THE TIME OF BEREAVEMENT

If a death occurs at home, contact the surgery immediately. A doctor will need to certify death and will inform you of the arrangements for obtaining the death certificate. In certain cases of sudden or unexpected death, the Coroner will need to be consulted. If a death occurs in hospital, the hospital doctor will usually issue a death certificate. Once the death has been certified you may contact an undertaker of your choice who will inform you of further arrangements.

ZERO TOLERANCE

We strongly support the NHS policy on zero tolerance. Anyone attending the surgery who abuses the GPs, staff or other patients, be it verbally, physically or in any threatening manner whatsoever, will risk removal from the practice list. In extreme cases we may summon the police to remove offenders from the practice premises.
